RN - Case Manager - Part-Time - Days

Description
Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
Reports to: Director of Case Management
FLSA: Non-exempt
Part-Time 24 hours per week
Qualifications
Minimum Education
Associate's degree or RN Diploma in Nursing - Required
