Position Summary:

Responsible for the timely processing of inquiry and referral calls. Serves as liaison to referral sources, physicians, unit staff and hospital personnel involved with each transfer request. Enters call data into the facility's database, manages all e-fax application(s) to process incoming referrals and transfers requests in a timely manner. Effectively communicates with physicians, unit and the admissions triage staff with assigned area and/or hospitals. Follows all hospital policies and procedures regarding the referral management process. This position requires flexibility in scheduling and offers opportunity for a hybrid work environment.

Essential Job Duties:

Receives all referral/inquiry calls and responds to requests for transfer providing patient assessments in a timely manner.

Ability to effectively address at risk, emotionally disturbed or in crisis potential patients and individuals effectively via telephone or via telehealth.

Enters all-call data into the facility database, ongoing utilization of the facility's electronic bed board for bed management purposes and completes the facility's e-fax application(s) to disposition incoming referrals in a timely manner.

Ensures all referral patient medical record information is completed and nursing and clinical staff are adequately apprised of the patient's condition/history. Ensures accurate information is provided to referral sources as requested.

Ability to effectively serve and work with patients, families, referral sources, community resources as well as departmental and facility co-workers and management teams.

Ability to work a schedule based on departmental and facility needs which may include day, evening and weekend shift hours.


Qualifications

Education:

RN License or Master Degree required

Knowledge:

1-3 years of experience preferred in psychiatric health care. Advanced knowledge and understanding of psychiatric services and medical capabilities that are provided by hospitals.

General knowledge and experience with reviewing medical and clinical information with appropriate medical staff and physicians. Working knowledge of involuntary processes and procedures and complies with all EMTALA regulations.

Skills:

Excellent customer service and communication skills (i.e., verbal and written). Ability to prioritize; work well under periods of high stress and pressure. Ability to demonstrate critical thinking skills and achieving prescribed goals.

Travel Requirements:

Up to 30%
